NEW PATIENTS

Your first visit to Dental Specialty Group establishes a vital foundation for our relationship with you. During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor. To understand what to expect for your first visit to our practice, please read through this page. You'll find all the practical information you need, such as a map and directions to our office, practice hours, payment policies and more. There's also background information about our committed staff and our first visit procedures. You can even save some time at your first visit by printing out and completing the patient forms in advance of your appointment.

Mission Statement
Our purpose, as a dental team, is to provide the best quality dental care to our young patients in an exciting child friendly environment where children are treated with respect and as individuals. We believe that with patience and perseverance, most children are capable of dental treatment without the use of heavy sedatives. But our primary goal is prevention and we strive in educating our patients and their parents about good oral hygiene and nutrition. We want to help children achieve excellent oral health, with the goal of a lifetime of smiles. We encourage and recommend that parents brings their children at one year age for a dental check up.

What to Expect at your first visit
We realize that time is very important in a parent's busy schedule. We reserve a reasonable amount of time for each patient visit and don't schedule large number of patients in each appointment.

We offer individualized dental care, focusing on the needs of the individual patient. This allows us time to listen to parent's questions rather that rush through an appointment. If the patient needs extra time to relax, get numb or play, they've got it! If parents have extra questions at the end of a procedure, we encourage them to ask. We try to be a "parent & kid friendly" office.

We also welcome emergency visit 24 hours a day / 7 days a week. As parents, we understand that children with a toothache or a dental accident can not wait for next day appointment. Please do not hesitate to call us as soon as your child needs to see us.

Your Initial appointment will consist of a consultation explaining your diagnosis and treatment options. A complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on another day.
Please assist us by providing the following information at the time of your consultation:

  • Your referral slip and any X-rays if applicable
  • A list of medications you are presently taking

If you have medical or dental insurance, bring the necessary completed forms. This will save time and allow us to help you process any claims.

New Patient Forms
We realize that filling out all of the new patient paperwork when you arrive to your appointment can be really time consuming, and can often set an appointment back. We are offering you the ability to download your new patient forms, with the following two options:

  1. Option 1 allows you to download, print, fill out, and bring your new patient forms in to your first appointment.
  2. Option 2 is an on-computer "fill in the blanks" type form that allows you to e-mail your information back to us after you finish, relieving you from having to remember to bring your paperwork in. How does it work? Download the forms entitled "FILL OUT ONLY", save them to your computer, and fill them out at your leisure. When you have finished them, attach them to an e-mail back here to us at: contactus@dsgconcord.com. When you come in for your appointment, we will have you sign the documents on our e-pad, attaching your digital signature to your paperwork.
